EOL’s quality work means you spend less time on adjustments – saving you valuable chair time

Our Guarantee

We will remake any appliance that does not fit the original model it was made on free of charge. All that we ask is that you return the appliance with the original model and a new impression. Please provide explanation of why it does not fit.

Also free of charge we will repair any appliance during the first 90 days of wear, not caused by patient abuse.

Turnaround/Shipping

  • 1 business day in lab time on RUSH appliances
  • 3 business days in lab time on non RUSH appliances
  • 6 business days in lab time on splints and aligner trays
  • Free daily anywhere in Georgia pick ups by an EXPRESS statewide courier
  • Free UPS or US Mail shipping to our lab for out-of-state customers
  • Guaranteed UPS Next day shipping back from our lab at minimal shipping fee. Call 678-455-3613 to have a UPS label faxed to you for rush out-of-state cases.
  • Free appliance case with each removable appliance
  • Each appliance is disinfected and packaged for your patient’s protection

Billing

Invoice: Each box is returned to you with individual invoice

Statement: Monthly statements will be sent out on the last day of each month

Payment Terms: Next 30 days from statement date

Past Due: All payments past 30 days from statement will be charged 3% on your next statement

Methods of Payment

Checks are preferred, but we also accept all major credit cards for a small handling fee.
